By registering my child(ren) to participate in Naples Soccer Academy (NSA) programs, I acknowledge and agree to the following:

1. Assumption of Risk

I understand that participation in soccer activities involves physical exertion and carries a risk of injury. I voluntarily assume full responsibility for any risks, injuries, or damages, known or unknown, that may occur as a result of my child(ren)’s participation.

2. Medical Authorization

In the event of an emergency, I authorize NSA staff to provide basic first aid and/or seek medical treatment for my child(ren). I understand that I am responsible for any resulting medical expenses.

3. Liability Release

I release and hold harmless Naples Soccer Academy, its coaches, staff, volunteers, and affiliates from any claims, liabilities, or expenses arising from participation in training, practices, games, clinics, or related activities, except in cases of gross negligence or willful misconduct.

4. Code of Conduct

I agree that my child(ren) and I will follow NSA rules, respect coaches, officials, and other participants, and demonstrate good sportsmanship at all times. NSA reserves the right to suspend or remove any player/family from activities for misconduct.

5. Media Release

I grant permission for NSA to use photos, video, and audio recordings of my child(ren) for promotional, educational, and marketing purposes (e.g., website, social media, flyers). If I do not wish to grant this, I will notify NSA in writing.

6. Refund Policy

I understand that registration fees are non-refundable, except in special circumstances determined at the sole discretion of NSA.

7. Agreement

By submitting this registration form, I confirm that I have read, understood, and agree to these Terms & Conditions.
